keep your HP above 0. well before 20ish mins all ways have at least one hp pot on you. and when you die get armor/MR depending on what damage you take more of.

The strongest offense and defense in LoL is positioning. Play all champions, learn their ability ranges and play styles. Learn to dodge skillshots. Learn to throw skill shots without telegraphing your intentions. Armor and MR aside, proper positioning and knowing your opponents will keep you alive and killing longer.

Just so you know, dieing is not the worst thing, it would be better if you stay alive. But if you sacrifice yourself so your team can just clean up 3 or more of their team, then that is a great trade off. Above all though don't be stupid, don't just chase one person down alone, because you are probably being led into a trap. Don't Turret dive, when you know the turret will focus on you and they have a stun, etc.

So, just play smart, and you shouldn't die more than 3 times a game depending on what role you play. Less if your a carry, more if your a tank

My number one Tip is
Don't try to help a teamate that has been ganked 4 v 1 because they over extended. one death is better than 2.

2: Get good tanks on your team, stay behind them and pick people off at leasure

Ward your own jungle, the river, and/or the enemy jungle. Buy wards and use them liberally. Knowing where the enemy is at all times is the best way to not die.

Restrain yourself from being reckless. Play slightly defensively even when you have the advantage. Do not over extend when you don't have your team with you, and do not get greedy with tower diving.

Be smart when you engage. Position yourself correctly in order to maximize output and survivability. You could have the best items in the world and they don't do anything if you're the squishy rambo who thinks he can flash in and kill the enemy team alone. Team fights take only a handful of seconds and they are what really decides games, and most games end because one or two team members get caught out of position, and that's gg.

To start, never run into the lane at the start of the game unless you know you can handle being jumped by 2 players. Wait at the far end of exp range until you know what you're up against.

Once you know who's in your lane, know what they can do. For example, stay away from creeps if malzahar puts his DoT on them, because it will bounce to you. If they have a skill shot ability (something that usually hits creeps if they get in the way), stay behind the wave.

Secondly, map control. This takes a while to grasp, but no good player relies on his team mates to call a missing in their lane, just watch the map yourself. I mostly glance at the map every oppurtunity, because the easiest way to die in the lane, is someone surprising you from the bushes when you have pushed too far.

Defensive play should always be employed, regardless of how much you are dominating them. Once you get a little better, you know when it's a good idea to risk yourself for a kill.

When you start a game, most characters will start with a build that includes at least one health potion. Either that or a doran's shield. These items will make laning easier from the get-go.

In the lane, when you are on half health or below; assess the enemies ability to get you. For example, if you're ezreal on 30% in the lane against a cho'gath, go back. why? because one stun will end with your demise, and if he's a good cho, he will get you, whether you're hugging the tower or not.

As someone said previously, positioning is VITAL. if you dash into the middle of the enemy team in a teamfight, you could get swept away by AOE or someones nuke. If you're in a good position, you should be ready for anyone who wants your life. If zilean suddenly judders towards you in a fight, expect a bomb, or a slow, either of which could screw you.

Never tower dive if the enemy team has something to stun/snare you, because that's just plain stupid, even if they are on low health.
