For the most part it refers to 3 items in the game that give you "stacks" each time you get a kill or an assist. Each stack makes the item better. A snowballing champion is a champion that has one or more of the items, and is building up stacks.

Mejai's Soulstealer = Ability Power
Sword of the Occult = Attack Damage
Leviathan = Health

Basically if the enemy team "snowballed".

They got an early advantage by first blooding or winning an early dragon fight.
This causes them to be stronger, causing them to win subsequent fights.
Eventually there team is simply way stronger than your team.

= You lose.

There are also champions that are good at snowballing. Champions that have a great early game and rely on taking advantage of that early game to be good end game. Champions like Pantheon, Mordekaiser (havent played him much after the change though, but pre change Morde was the most extreme case of snowballing champion) err Shaco also kinda, relies on kills through early and mid game.
You get the point, right?

Snowballing in general gaming terms, is someone who starts off well and exponentially gets out of control. In this game, it most often refers to Mejai's Soulstealer, Sword of the Occult, and less-so Leviathan(since stacks of health often won't make you spiral out of control, unless you're Shen). I've also seen people reference snowballing when getting bloodrazors or ganking early to amass lots of gold, but as in most games, people use the term in various ways.
