Ok stacks refers to multiple things:

For TWITCH: His passive is poison (hover over the small box to the left of your skills)
>Every time you attack the thing you attacked gets a poison stack.
>Every poison stack causes damage over time to that enemy.
>His abilities are more powerful the more poison stacks the enemy has on them.
Therefore, attack multiple times if you can before using your abilities.

There are some items that also give stacks: leviathan, mejais soulstealer, etc.
>These items give you 2 stacks for every kill you get, and 1 stack for every assist you get.
>They give you bigger bonuses the more stacks they have.
>Part of your stacks are lost every time you die.
These are referred to snowballing items, because the better you do the more powerful you get, which makes you do even better.

Other champions also have a stack mechanic which can do different things.

Stacks are counters for items that give bonuses based on how many times you've done something, like kills/assists/deaths or hitting someone. For instance, take a look at Sword of the Occult (http://www.leagueoflegends.com/items#3141):

+10 Attack Damage UNIQUE Passive: Your champion gains 5 damage per stack, receiving 2 stacks for a kill or 1 stack for an assist. This effect can stack 20 times; you lose a third of your stacks if you die. At 20 stacks, your champion's movement speed is increased 15%.
When you kill someone, you get two "stacks". At 5 damage per stack, that's +10 damage added to your item for +20 total. When you assist, you get one stack. When you die, you lose a third of your stacks.

Look at Guinsoo's Rageblade (http://www.leagueoflegends.com/items#3124):

+35 Attack Damage +45 Ability Power Passive: On attack or spell cast, increases your Attack Speed by 4% and Ability Power by 6. Lasts 5 seconds (stacks up to 8 times).
Any time you attack or cast a spell, you get 1 stack. Each stack gives you +AS and +AP. You lose ALL your stacks if you don't attack/cast within 5 seconds.

Some other items that stack are Mejai's Soulstealer, Leviathan, The Bloodthirster, The Black Cleaver, etc.

EDIT - Why would you rate this post down? Is there something incorrect in here??

As stated, stack serve many purposes

Debuffing/Damaging stacks
-Twitch's poison for example deals damage per second per stack (the more stacks, the more damage per second) Also, expunge deals more damage based on the number of stacks you have.
-Other debuffs are like Malady or Black Cleaver which lowers armor or MR based on how many hits you take.

Buffing Stacks that can be lost:
-Mejai's, Sword of the Occult, Leviathon [Kill champs gain two stacks, get and assist gain 1]

Sort-of Stacks
-Bloodthirster [this gives you a stack when you kill ANYTHING] the stacks on the BT increase your lifesteal AND damage
-Warmog's killing minions and champs grants you more health and more regen. These are not lost upon death.
