Yep, I'm not going to pretend I have enough info to make exact balancing modifications to any items, that sort of thing is certainly the domain of Xypherous and Co. I'm sure the exact numbers for Caustic Cleaver would need tweaking, I'm glad you like the concept though!

Rather than the revolver, you should use the Haunting Guise.

It would give that item a better use, as well as an upgrade path, since revolver already has an upgrade.

If you scroll up a bit on the first page you'll see my reply to this. In essence I agree with you, but I don't think health is a good stat for Caustic Cleaver. It could be too abusable on bruisers and even ADs to some extent. Spell vamp seems to be better suited as it add to survivability somewhat, but only if you are willing to be offensive and use your abilities to cause damage.

For example, I thought one of the roles for this item could be as a soft counter for some ADs to diving bruisers, e.g. Kog'Maw versus Irelia. By giving a small burst of sustain from spell vamp as well as the increased damage/burst (mainly burst as the MagPen will affect abilities for many champions more than ArmPen) from the pen/AD/AP. If you take exhaust and the mastery point, have a little bit of peel from your team and use a Caustic Cleaver you might stand a bit more chance of surviving the dive by actually retaliating and killing the diver. This scenario is of course already possible for some ADs, but in most games unless the AD is very significantly ahead or has a lot of help from their team they will not survive unless they have a reposition available, and even then, the dive itself will have prevented them from dealing any damage in the team fight.

Clearly tactically there are counters to this problem already, but they typically involve better team work and defensive itemisation, which is often less than ideal in a soloque environment. E.g. Enemy Irelia got first blood on your top, proceeds to zone and dominate top lane, all your other lanes did well and won albeit marginally, but you have no chance in team fights as the Irelia can shut down your carries so well. I'm not saying emphatically winning your lane should not be rewarding (even though top lane is so swingy it's verging on broken), but I do think diving head first into the enemy team on any champion should be much more of a balance of risk/reward than it is currently. E.g. Malphite/Irelia who can just stick to and shutdown completely (if not kill) almost any AD they chose and probably survive too in the absence of any concerted effort from the team to peel effectively.

Large gap closers are fun and yield exciting gameplay, but the AD should have some more options in terms of offensive items that allow them to counter this (particularly if they have been laning well and are ahead of the curve). Of course I don't want this item to be so powerful that ADs will be able to nuke anyone that comes for them in a team fight without breaking a sweat and ending up with full health afterwards, just to skew this interaction slightly more in favour of the AD so that 'insert diving bruiser here' at the very least doesn't get to walk away from the 'dive' almost unscathed.
