Smartcast means you don't have to click your mouse.
Your ability will target whatever your mouse cursor is hovering over when you press the hotkey.

So, basically, you have a Hextech Gunblade at hotkey 1.
You hover your mouse over an opponent, and press 1, and that will suffice.

Many players use smartcast for all their skills, which greatly improves response time. Personally, I can't play without smartcast, so I'm quite happy that I can now smartcast items.

I was messing around with Eve in a custom with some friends earlier.

I got a gunblade and DFG (yes, like I said, messing around).

I bound them both to smartcast, stealthed, walked up to the enemy Caitlyn or Riven (since neither had any MR), then hit both the bound keys, plus E, plus F for ignite. Instant explosion with no chance to react.

If I had to go 1-click-2-click-E-click-F-click they might have been able to flash away in the middle of it all.
