I think items need a serious rebalancing...
These are my suggestions, and are by no means the correct thing to do...
Just my personal views on them...

Frozen Mallet
- ridiculous effect with health tagged on to it
- change belt of strength to cloth armor and null magic mantle
- up the recipe cost to 1275 (+450), total cost change = 0, stat change -275hp +20ar +25mr

Warden's Mail and Thorn mail
- can now combine into Warden's Thorn Mail
- has both the stats of Warden's Mail and Thorn Mail, with a 800g recipe

Cloak of Agility
- too much crit, nerf it to 12% crit chance from 18%
- indirectly nerfs atma's impaler, infinity edge and phantom dancer

Stark's Fervor
- so much aura boost for so little money, hp regen aura even rivals innervating locket
- attack speed aura now only limited to self
- hp regen aura now at 12hp per 5 second, only slight buff from Emblem of Valour
- recipe cost increase to 1200 from 700
- see recurve bow nerf below

Warmog's Armor
- change way of stacking the ability
- Permanently gain 4 Health and .5 Health Regen per 5 sec per 100 damage taken.
- Permanently gain 8 Health and 1 Health Regen per 5 sec per 125 magic damage taken.

Bloodthrister
- change way of stacking the ability
- Gain an additional 1 Physical Damage and 0.25% Lifesteal per 200 damage dealt

Recurve Bow
- so much attack speed for so little cost
- nerf to 25% attack speed from 40% attack speed
- indirectly nerf's last whisper, stark's fervor, wit's end and bloodrazor as well

Executioner's Calling
- now stacks with diminishing returns with other hero abilities, like Gangplank and Katarina
- now properly works as anti-lifesteal or healing item on certain heroes

Rod of Ages
- Again, powerful combo of attack and defense in the same item
- nerf, -100hp, -100mana, -20ap, still gain 300hp 300mana 30ap over 15 minutes
- includes latest Catalyst nerf

Wit's End
- Can now combine into Grand Wit's End
- combines 2 Wit's End together into 1
- Unique effect: burns 3% of targets current mana

Malady, Guinsoo's Rageblade and Tiamat
- can now combine into Guinsoo's Flameblade
- Has all the effect of Malady, Tiamat and Rageblade, with 1000g recipe
- Unique effect: deals 50 bonus AOE damage when Flameblade is fully charged from rageblade ability
- An alternative ultimate varied item to Tri-Force

Banshee's Veil
- so much survival on top of the spell shield
- change Negatron Cloak to a weaker null magic mantle
- increase recipe cost to 990g from 650g. total cost change = 0
- includes latest Catalyst nerf

Atma's Impaler
- buff damage to 3% of your maximum hp

Brawler's Glove
- nerf crit to 6% crit chance from 8%

Berserker's Greaves
- buff, +3% critical chance, +3% life steal

Sorcerer's Shoes
- buff, +10AP

The Catalyst nerf was a nerf to both Banshee's Veil and RoA. The reason those items were so good was that they required an item that was basically mandatory for so many champions. Now that there is less incentive to build Cat, the items that it build into will be less attractive to buy. Also, your hating on crit chance seems bizarre to me. And, given the hard cap on IAS nerfing recurve bow is hardly necessary. It seems to me you like to play casters (you haven't nerfed a single AP item except for RoA) and want to reduce the power of carries. The thing is, the game is balanced around the endgame power of physical DPS. If you like casters you need to leverage their earlygame advantage into a faster win. Carries only get ridiculous if they are allowed to farm money for all of the items that you want to nerf. So, harass and gank them earlygame when your abilities deal far more damage than theirs and you'll have prevented them from buying all this gear. Finally, you give no reason for your proposed stacking changes to Bloodthirster and Warmog's, what is wrong with how they work now?

I am glad the OP does not balance the game. His changes are awful!
