it isnt that the item itself is bad, but the fact that you could be rushing for a better item such as sword of the occult/IE/LW/Cleaver

Here is my build (i play twitch fairly well):

Dorans shield (as people said earlier it gives you armor, hp regen, and hp)
Merc treads or ninja ( you can go greaves if you want, but you really dont need it)
Last whisper (recurve bow first)
Inf Edge (BF always first)
Blood thirster or starks (BF first for blood thirster)

Game usually ends by now if not before, but if it keeps going i get:
Phantom Dancers (some speed)
Get whatever else you need (survivability, damage, speed)

Some tips are to always be behind the enemy before you start to attack, because if they run forward they will go into the tower .
Get W before lvl 6 because it helps to slow them to get an easier kill.
In team fights hit Q,R, and then E and W while they are running. You might want to hit E if you see enemys on low HP because they have probably been hit by your ult and that will give you and easy kill. Hit W when they start to run so your team can finish them.

Hope i helped :P

With Flash, you can chase and escape. With Cleanse, you can only escape.
You need Cleanse because any good team will save an Exhaust for when you pop out in a team battle, and then suddenly you're worthless to your team. This is why nearly every physical dps needs Cleanse.
