It's purpose is more for shredding high health mages and whatnot, not necessarily tanks. That's what resistance penetration is for.

How about +0.5% of enemy max HP increase every hit, and stacking 6 times to 3%? This stacks on a single target for 5 seconds and is removed when this is applied on a new target.

And then a bit of armor/AD tweaks.

Edit: Or the MR shred, what if we built it from a malady/sword instead of recurve bow/pickaxe?

Bloodrazor is meant to counter high high health builds specifically, not actual tanks.

Honestly, any build or champion that is built to specifically to counter actual tanks and kill them fast, Amumu, Allistar, Rammus, Leona, etc could be considered unbalanced because it would make them incapable of doing their job.

It would also kill non-tanks ridiculously fast, which would cause problems unless it was based on some broken mechanic like dealing true damage based on the sum of enemy armor and magic resistance or something. (We don't need more true damage in this game btw, less would be better).

Bloodrazor could use a slight cost decrease though. It is rather expensive.

imo, the shred or penetration + a damage tweak and a little bit of cost reduction would be a nice change.

Remove Pickaxe form cost. MBR is now an excellent item. If absolutely needed bump % damage to 5%

I'd agree maybe it should be a hair cheaper, but as an item, it dishes out incredible damage, it doesn't need buffed damage.

NO. True Damage based on % HP will not happen on an item, EVER. It's bad enough how much people complained about Vayne (who was near perfectly balanced on release) because her W added a bit of true damage and allowed her to kill a target in 3 hits instead of 4, also allowed her to harass tanks and champs with Thornmail.

Plus, Kog'maw and Warwick would be OP as hell with this. Kog'maw already gets to 10% HP damage with his BaB and Madreds, and then Warwick's ultimate applies on-hit effects to 5 hits (adds up to 20% HP damage). Not to mention the fact that it's a great DPS item for having AD, Armor AND AS all at once.

Madred's problem is not that it's UP, it's that it's not cost efficient currently. Drop Pickaxe from its combine, make the MBR AD 20, there we go. Now it costs 2825 instead of 3800 gold. Maybe even less if they drop the combine cost a bit.
